This tool is based on the Patient Health Questionnaire (PHQ-9), a standard, multipurpose instrument used by clinicians to screen for, diagnose, monitor, and measure the severity of depression.
Instructions:Consider your feelings and thoughts over the last 2 weeks. For each item, select the option that best describes how often you have been bothered by that problem.
Scoring Guide:The total score helps to categorize potential depression severity:
- 1-4: Minimal depression
- 5-9: Mild depression
- 10-14: Moderate depression
- 15-19: Moderately severe depression
- 20-27: Severe depression
